Placing an order:

- Save the form to a place you will remember on your computer
- Complete the form using MS Excel and save your changes
- You can either e-mail us the form or print and mail/fax it to us
Retailer Order Forms
All new accounts are prepaid by credit card or PayPal. A 50% deposit is required at time of order placement. Final payment plus shipping costs is due prior to order shipment. Please save and fill out our credit card authorization form, or pay online through PayPal. Returning clients can request net 10 – 30 day term account pending credit application approval and personal guarantee.
All sales are final. We will send item mock ups or proofs on all custom jobs for your approval prior to production. You can also request a mock up be sent for all service bureau work. It is your responsibility to check the mock up for ANY and all typographical, color, layout, size and style errors. It is recommended that you check your order immediately upon arrival. If any error, not mentioned on the mock up occurs, then we will offer to replace or discount the order within 30 days of receipt.
